This form is to gather contact information and to obtain agreement by you of your responsibility and obligation for being on a committee.

The committee convenes twice annually to discuss and award scholarships to high school and continuing college students. One meeting is held at the beginning of the calendar year, and the other is held in the Summer. Attendance may be in person or by conference call, and is held at CFT’s Burbank office. The committee member is required to read and review applications, and provide a score to each section. This process is all done online through a website managed by CFT. Additional work may include research, policy development, as well as other needs as defined by the committee.
